Lucky Healthcare Services, Inc. (HCS program) provides
its services to individuals with developmental disabilities. We
have a non-denial policy in enrollment in HCS program. To receive
our services, the individu
al
must have have the following:
Eligible for SS1 and Medicaid
A current level of Care and x117 form
A current medical examination
A determination of mental retardation (DMR)
A verification of informed choice
